The Heart of Chosen Family
“You are not too complex. You are not too hard. You are not here to be fixed. You are whole — and you deserve support that sees your strength, honours your story, and walks beside you on your terms.
At Chosen Family, we centre you — your voice, your culture, your priorities. We work through the lenses of intersectionality, person-centred care, trauma-informed practice, and Ubuntu — because we believe your healing, empowerment and belonging are collective responsibilities.
Whether you’re navigating the NDIS, disability, faith, culture, class, race or LGBTQIA+ identity — you don’t just deserve to be accepted. You deserve to be welcomed.
This is support done differently. This is Chosen Family. Chosen Family is my answer. It’s a place where care is personal, connection is meaningful, and every participant is empowered to take control of their story.
I invite you to join Chosen Family and feel seen again.”

About us
Transforming your NDIS Experience
At Chosen Family, we’re committed to co-designing your future by truly listening to you and creating care that reflects your needs and aspirations. We focus on community, inclusion, and joy, ensuring that those who have felt left behind are finally seen and heard. Together, we’ll build a life shaped by your voice, empowering you to take charge of your journey.
Flexibility
Recognising that the diverse needs of our participants defy a one-size-fits-all solution we find ourselves in a constant pursuit of flexibility. Frequently the systems designed to aid our participants lack the necessary resilience inadvertently leaving our participants exposed to the potential harm these systems can inflict instead of fostering the support they so rightly deserve. We hope to ensure that our procedures and service delivery are not instruments of harm but rather become the steadfast pillars of support our participants can always rely upon.
Tenderness
Embracing the guiding principle of tenderness within our team of support workers enables us to not just acknowledge but wholeheartedly honor the innate humanity of our participants. It embodies the act of approaching and engaging with individuals in a manner imbued with gentleness care and genuine compassion. This practice cultivates a nurturing and harmonious atmosphere fostering profound connections and mutual understanding among individuals. In these moments of tenderness, we create a space where the human spirit can flourish.
Equity
We are committed to fostering an inclusive and equitable environment where every individual, regardless of their background, identity, or experiences, has equal access to opportunities, resources, and recognition. We celebrate diversity in all its forms, including race, gender, ability, socioeconomic status, and sexual orientation, with a particular emphasis on uplifting and supporting LGBTQIA+ individuals. By championing fairness and creating a safe, welcoming space for all, we ensure that every voice is valued and empowered to thrive.
Belonging
We create spaces where everyone feels seen, heard, and valued, cultivating an environment where individuals can fully express themselves and thrive. At Chosen Family, belonging means embracing each person’s unique story, fostering deep connections, and building a community rooted in trust, acceptance, and mutual respect. Together, we ensure that no one stands alone, and everyone feels at home in their journey toward self-determination and success.
Mission
We envision a future where every participant can live confidently, free from barriers and biases. By fostering connection and belonging, we create communities where everyone feels valued and supported, and every individual can grow, contribute, and live with dignity.
Vision
We are here to co-design your future, transforming disability services with the strength of family and the power of community. Together, we create spaces where everyone belongs, where your voice shapes the care you receive, and where your future is crafted with compassion.

Social Work Model
Understanding the Social Model of Disability
Chosen Family subscribes to the social model of disability, developed by people with disabilities. It asserts that individuals are disabled by societal barriers, not by their impairments. These barriers can be:
- Physical – like inaccessible buildings
- Institutional – discriminatory policies or systems
- Attitudinal – such as low expectations
- Communication-based – inaccessible formats or complex messaging
This is distinct from the medical model, which focuses on fixing the individual, often leading to reduced autonomy.
Common Barriers Faced by People with Disabilities
Attitudinal Barriers
Created by societal perceptions that reduce people to their disability—this includes bullying, discrimination and low expectations.
Environmental Barriers
- Narrow doorways or path
- Inaccessible desks
- Poor lighting
- Difficult doorknobs
Institutional Barriers
- Lack of reasonable adjustments
- Inaccessible transport
- Discriminatory policies
- Communication Barriers
Examples
- Small print or no Braille
- No captions or Auslan
- Complex, technical language without Easy Read alternatives
Our Approach at Chosen Family
- More independence
- Greater choice
- True community participation
This commitment helps us foster a society where everyone has equal opportunities and can make their own decisions confidently.

Ubuntu Framework
The concept ubuntu comes from the Zulu and Xhola languages and can be roughly translated as “humanity towards others,” and “the belief in a universal bond of sharing that connects all humanity.”
During the 1990s, the concept of ubuntu was adapted as an ideology by post-apartheid in South Africa was used as a vehicle to bring about harmony and cooperation among its many racial and ethnic groups. The ethical values of ubuntu include respect for others, helpfulness, community, sharing, caring, trust, and unselfishness. Ubuntu is an African philosophy that places emphasis on ‘being human through other people’. “Premise that “I am” only because “we are.” “Ubuntu is rooted in what I call a relational form of personhood, basically meaning that you are because of the others,”. Your humanity, your personhood—you are fostered in relation to other people.” Practically it is to look into another person’s eyes and have that person return the gaze. When the gaze is returned, that recognition is what humanizes you.
As you think about harmony and health, know it begins within you. We must allow light to inhabit the places within us that have been colonized by shame, misinformation, fear and repressed rage. Brené Brown writes that “true belonging doesn’t require us to change who we are; it requires us to be who we are.”
