Supporting others has always been part of who I am. From a very young age, I was naturally drawn to helping people and making a difference where I could. Growing up, I saw firsthand how families and individuals with disabilities can struggle without the right support—and that stayed with me.
At 13 years old, I began volunteering with Riding for the Disabled, which sparked a lifelong passion. I went on to become an RDA coach and later supported a close friend—someone I once coached—who competed at the RDA Championships in Melbourne with my personal horse. That full-circle moment confirmed what I already knew: this is my purpose.
Over the past 15+ years, I’ve worked across Supported Independent Living (SIL), direct support, and support coordination—progressing from Level 2 Support Coordination through to Level 3, and into leadership roles as a Team Leader and Manager. This journey has given me both hands-on experience and strategic insight to support even the most complex situations.

Supporting others has always been a core part of who I am. From an early age, I was raised with strong values around helping people, and I have always believed that the right support can make a real and lasting difference in someone’s life.
Although I’ve worked across a variety of industries, it was in this sector that I found where I truly belong. Over the years, I’ve developed broad experience as a Local Area Coordinator, Support Coordinator, Specialist Support Coordinator, Recovery Coach, and in leadership and management roles, giving me a strong and balanced understanding of both direct support and the bigger picture.

I'm an experienced Support Coordinator with a background in child protection, mental health, psychosocial disability and intellectual disability. I have supported people of all ages and managed complex caseloads across frontline and leadership roles.
I'm passionate about building genuine relationships and helping participants overcome barriers, access the right supports and work towards their goals. With a hands-on, straightforward approach and strong knowledge of NDIS plans, funding, risk management and service coordination, I provide clear, practical guidance that helps make the NDIS feel less overwhelming.

Mel brings a compassionate, real‑world approach to support coordination, backed by over seven years of hands‑on experience across disability, aged care, and case management. She works alongside participants and families to simplify the NDIS, build confidence and create practical pathways toward independence and stability.
Mel is a mum who brings patience, understanding and a calm, steady approach to her work. She handles pressure well, multitasks with ease, keeps things organised and on track. Mel genuinely cares about people feeling safe, supported and not alone and believes the small wins matter just as much as the big ones. Her background also includes hairdressing, childcare and administration, shaping her practical and down‑to‑earth style. Outside of work, she enjoys fishing, art, making and reading.

Kia has a masters in psychology. She supports individuals with complex needs, behaviours of concern, and trauma backgrounds. As a neurodivergent person and carer, she brings both clinical insight and lived experience to her work. Her practice is person-led, focused on reducing harm, protecting autonomy, and building practical strategies that align with each person’s values and goals.

Hi all! My names Joshua. Brand new Support Coordinator jumping into the Chosen Family team.
Little bit about me I have constantly lived abroad due to family and have bounced between Australia and Canada on numerous occasions and now calling Sydney home.
I have a big heart for kids and teens and have work extensively in areas such as Youth Work, Child Protection and school programs.
Prior to moving to Sydney my family and I are kinship carers and have been since 2022. I moved into the Support Coordination space to help make a bigger impact on the community and aid those who may or may not have a support network readily available to them.
I pride myself on rapport I hold with my clients and team and strive to make my work as fun as possible and to make my clients enjoy the time they have with me.
Outside of work I’m your typical gym junkie, I gym 6x a week and often do so with my partner or mates.
I am a big video game nerd and love all things JRPG and RPG so please feel free to chat about those as I could easily chat for hours about video games. If I can say so I am a mad cook, love to experiment and try new ingredients, cuisines, and cooking styles. Unfortunately they don’t always fit into my daily macros but hey if it tastes good then go for it.
